What forms need to be completed and when are they due?
All required forms and registration needs to be completed online via FinalForms BEFORE an athlete can participate in practices: Here is the link : https://www.fhcathletics.com/main/filesLinks/
Athletic Physical -must be dated on April 15 or later of the upcoming season to be good for this school year. In other words, complete your physical every year AFTER April 15 but before August. BRING THE FORM to the physical as it needs your doctor’s signature. Completed physical forms can be uploaded in FinalForms online.

Participation Form and Fee - the $140 participation fee and form is due now.
Players will not be allowed to practice if the parent and athlete information/signatures are not complete in Final Forms.

What equipment do I need to buy?
Cleats you would purchase.
Mouth guard - A mouth guard is provided. Unless players want a special mouth guard which would be on your own.
Gloves - Gloves are on your own.
Spirit pack items (usually a t-shirt and shorts) from a website link that will be provided to you. You can buy extras or additional products from the site.
School Provides:
Helmet, shoulder pads, mouth guard, girdle, practice and game jerseys and game pants.
Helmet - YOU DO NOT HAVE TO PURCHASE A HELMET. For those who do want to order a helmet, the Riddell rep will be available early in the summer lifting season to measure for helmet size. He will let your player know what size to order then you will need to go on a special website link and purchase the helmet.
How can I keep updated with practices and the game schedule?
This website will be the most updated schedule and information available. If any urgent issues arrive your team parent or coach will communicate via email.
